This web site, herein referred to as the "Site", is owned and operated by His Majesty in Right of Alberta, as represented by Primary and Preventative Health Services (“PPHS”) and the Health Shared Services Provincial Health Corporation (“HSS”). By accessing or using the Site you, the user, agree to these Site Access and Use Terms and Conditions, as they may be amended or updated from time to time (the “Terms and Conditions”). If you do not agree to all of the Terms and Conditions, you must immediately discontinue your access and use of the Site. If you fail to do so and continue to access and use the Site, you are deemed to have accepted these Terms and Conditions.
In these Terms and Conditions, PPHS, HSS and their agents, suppliers, service providers and other representatives are all together referred to as “we” or “us” or “MyHealth.Alberta.ca”.
“You” or “your” or “user” means any individual who accesses, browses, or uses the Site, including on behalf of a company, organization, or other entity. If you are using the Site on behalf of such an entity, you represent that you have authority to bind that entity to these Terms and Conditions.

General
These Terms and Conditions and MyHealth Alberta User Agreement represent the entire agreement between the user and MyHealth.Alberta.ca regarding use of the Site and its Content. In the event a court of competent jurisdiction finds any provision of the Terms and Conditions, Copyright and Disclaimer, or MyHealth Alberta User Agreement to be unenforceable, the remainder of the provisions shall be unaffected and continue in full force and effect. The failure of MyHealth.Alberta.ca to enforce strict performance of any provision of the Terms and Conditions, Copyright and Disclaimer, or MyHealth Alberta User Agreement shall not be construed as a waiver of any such provision or right.
You agree that your use of the Site shall not violate any applicable local, national or international law, including but not limited to any regulations having the force of law. If you violate any of the Terms and Conditions, Copyright and Disclaimer, or MyHealth Alberta User Agreement, your limited licence to use the Content will automatically terminate and You must immediately cease your use of the Site and destroy any copies of the Content or any portion thereof.
Jurisdiction
This Site is administered by PPHS and HSS, which are located in Alberta, Canada. Any matter arising out of the use of this Site shall be governed by the laws of the Province of Alberta and the federal laws of Canada applicable therein, without reference to its conflict of laws principles. You agree to submit to the exclusive jurisdiction of the courts of the Province of Alberta to determine any action or proceeding arising out of or in connection with this Site.

Termination
MyHealth.Alberta.ca may terminate this agreement, including any licences contained herein, at any time and further may terminate the user’s access to any and all portions of this Site if, MyHealth.Alberta.ca determines that the user failed to comply with these Terms and Conditions. Upon termination, the user must cease use of this Site and destroy all materials obtained from this Site, including all copies thereof, whether made under these Terms or otherwise, and/or take any additional or alternative reasonable steps that MyHealth.Alberta.ca may direct in respect of the termination.
Proprietary Right and Grant of Limited License
This Site and the Content are protected by copyright, trademark and other proprietary rights of MyHealth.Alberta.ca. Except as expressly permitted in these Terms and Conditions, no portion of the Site or the Content, including without limitation trademarks, trade names and logos, may be reproduced in any form, or by any means, without prior written permission from PPHS, HSS or the applicable third party. MyHealth Alberta and MyHealth Alberta Account User Agreement
MyHealth Alberta Account (“MHAA”) is a service, accessible via the MyHealth Alberta portal or app allowing access to a variety of tools including My Personal Records (“MPR”) and MyChart.
MPR offers Albertans access to their personal health information such as lab results, diagnostic reports, and medication information from Alberta Netcare, the provincial Electronic Health Record.
MyChart offers patients personalized and secure online access to their information related to care received within Connect Care, the clinical information system of Connect Care enabled facilities or providers, together with the communications, care management and health monitoring tools that can help them achieve their healthcare goals.
This MyHealth Alberta and MyHealth Alberta Account User Agreement (“User Agreement”) applies to the MyHealth Alberta Health Portal and the tools contained within MHAA. Additionally, use of MyChart is subject to a separate Terms of Use and Privacy Policy, which can be viewed here:

MHAA is operated by His Majesty in Right of Alberta, as represented by Primary and Preventative Health Services (“PPHS”) and HSS. Primary and Preventative Health Services, HSS, and their agents, suppliers, service providers and other representatives are all together referred to as “we” or “us” in this User Agreement.
In order to access a MyHealth Alberta Account, you must first create an Alberta.ca Account. This enables individuals to use their identity credentials throughout any Alberta.ca Account services, including MyHealth Alberta. You must become an identity-verified user, and have your identity confirmed through a verification process through Alberta.ca Account.

By accepting these terms and conditions you confirm that you are at least 14 years of age. We reserve the right to modify or change the terms of use at any time, which will be indicated by the date of the most recent change located beside the “Last Modified” date after this section. You will be required to re-accept a revised user agreement prior to using the system. If at any time you do not wish to be bound by the terms of use, you must immediately deactivate your MyHealth Alberta Account in accordance with these terms of use.
We may suspend connectivity between Alberta Netcare or other PPHS systems and the MHAA at any time and on immediate or no notice if we reasonably believe that such suspension is necessary (i) to prevent imminent threats to the privacy, security and integrity of information within MHA or to the performance or availability of MHA; or (ii) to comply with any applicable Canadian laws. We will use commercially reasonable efforts to promptly re-instate connectivity upon elimination of threat or potential non-compliance with applicable Canadian laws.
We also reserve the right to modify or discontinue (temporarily or permanently) this service (or any part of it) with or without prior notice to you. However, in most instances a notice will be sent to your account regarding large-scale changes 30 days prior to the modification. We are not liable to you or any third party for any modification, discontinuance, or suspension of MyHealth Alberta or MHAA.

MyHealth Alberta or MHAA is not intended for urgent matters or for use in emergencies. Do not send communications through MyHealth Alberta or MHAA that require urgent attention. If you have an emergency, call your health service provider(s) directly, go to the nearest emergency room, and/or call 911. User Registration and Login
You are solely responsible to maintain the confidentiality of your MyHealth Alberta Account and passwords you use to access the tools within and agree neither PPHS nor HSS has any obligations with regard thereto. You will be required to enter a valid password to access MyHealth Alberta and the tools within. It is your sole responsibility to monitor and control use of these passwords for all purposes. You accept all responsibility for the security of your passwords and use of MyHealth Alberta or MHAA via the passwords, including whether you choose to use MyHealth Alberta or MHAA in public places. You agree that you are solely liable for all authorized and unauthorized access using your passwords. You agree to keep your passwords strictly confidential and to not disclose them to anyone not authorized to act on your behalf. We are not liable for any loss or damage arising from your failure to comply with this section of the terms of use or for any unauthorized access to information you have entered or asked to be entered in MyHealth Alberta or MHAA. Please notify MHAA Support of any potential unauthorized use(s) of your account or suspected breach(es) of security.
Use of MHAA by Health Services Providers
MHAA was not designed to be used for the provision of health services. The records in MHAA may not be a complete or comprehensive view of an individual’s medical history or health records. The records in MHAA are not clinical records. We do not guarantee the accuracy of information in MHAA and assume no responsibility for how health service providers make use of the information obtained from MHAA.
Nothing in this User Agreement prohibits health service providers from registering for their own personal MyHealth Alberta Account and using that account for their own personal use.
Restrictions on Use
We grant you a limited license to make personal use only of the MyHealth Alberta Account, or to use it as a health services provider, in accordance with these terms of use. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale or other commercial use of the MyHealth Alberta Account, making any derivative of the MyHealth Alberta Account, the collection and use of user email addresses or other user information, including, without limitation, health information or any data extraction or data mining whatsoever.
You must not use the MyHealth Alberta Account for any purpose other than that for which it is designed and intended, as set out in this User Agreement. In addition, (i) unless otherwise expressly permitted in this User Agreement, you must not alter, tamper with or attempt to repair MyHealth Alberta or MHAA; and (ii) unless otherwise expressly permitted in this User Agreement, you are not entitled to resell MyHealth Alberta or MHAA.
You must not (i) license, sublicense, sell, resell, rent, lease, transfer, assign (except as expressly permitted), distribute, time share or otherwise commercially exploit or make the MyHealth Alberta or MHAA available to any third-party, other than to Users; (ii) remove, alter, or obstruct copyright notices, trademarks or other legends on the MHAA service, or its packaging, documentation or related materials; (iii) intentionally send or store: (a) spam or otherwise duplicative or unsolicited messages in violation of applicable laws, (b) infringing, obscene, threatening, harassing, abusive, slanderous, hateful, libelous, or otherwise unlawful or tortious material, including material that is harmful to children or violates third-party privacy rights, or (c) malicious or harmful code, including but not limited to viruses, worms, time bombs, Trojan horses and other harmful or malicious code, files, scripts, agents or programs; (iv) interfere with or disrupt the integrity or performance of the MyHealth Alberta or MHAA or the data contained therein; (v) attempt to gain unauthorized access to the product or its related systems or networks (vi) modify the MyHealth Alberta or MHAA or create derivative works (vii) create internet links to the MyHealth Alberta or MHAA or copy, frame or mirror any part of the MyHealth Alberta or MHAA (or any source code or object code thereof) (viii) reverse engineer, disassemble, de-compile, modify, translate, create derivatives, disassemble or use for competitive analysis the MyHealth Alberta or MHAA (ix) access, copy or use the MyHealth Alberta or MHAA in order to: (a) build a competitive product or service, or (b) copy any features, functions or graphics of the MyHealth Alberta or MHAA, or (x) use the MyHealth Alberta or MHAA to impersonate another person or infringe the intellectual property rights of any third party.
You further agree you will not use the MyHealth Alberta account or MHAA in any manner that violates provincial, national, or international laws. You will not infringe upon the intellectual property of others. You will not attempt to disable, spam, hack, or interfere with the proper function of MyHealth Alberta or MHAA. You will not upload viruses or malicious content, perform SQL injections, or hack the system in any manner.
Ownership of MyHealth Alberta and MHR
MyHealth Alberta and MHAA (excluding Your Additional MyHealth Alberta or MHAA Content), including but not limited to its contents and design are the property of PPHS and HSS, their subsidiaries, affiliates, vendors and/or licensors. MyHealth Alberta and MHAA are protected by Canadian and international copyright, trademark and/or patent laws. Any rights not expressly granted herein are reserved.
The contents of MyHealth Alberta and MHAA (excluding Your Additional MyHealth Alberta or MHAA Content), including without limitation the text, images, audio, and video, and any materials accessed through or made available for use or download through MyHealth Alberta or MHAA are copyrighted and may not be copied, distributed, modified, reproduced, published or used, in whole or in part, without the prior written consent of PPHS and HSS, except that you may download content from MyHealth Alberta and MHAA for your personal use only, provided you keep intact all copyright, trademark, patent and other proprietary notices and comply with any applicable end user license agreements. No part of any content, form or document may be reproduced in any form or incorporated into any information retrieval system, electronic or mechanical, other than for your personal use (but not for resale or redistribution).
For purposes of these terms of use, any use of these materials on any other web site is prohibited. You do not acquire ownership rights to any content, document or other materials viewed through MyHealth Alberta and MHAA. The posting of information or materials on MyHealth Alberta and MHAA does not constitute a waiver of any right in such information and materials. All software used on MyHealth Alberta and MHAA is the property of PPHS, HSS, or their software suppliers and protected by Canadian and international copyright laws.
Opening Your Account and Uploading Your Personal Health Information
MHAA enables the uploading and exporting of specific health information data types through the connection of personal health devices. You are solely responsible for safely uploading your personal health information.
You may enter, upload, store, and access personal health information, including copies of documents, records, and images into MHAA. You are responsible for choosing whether or not to enter or to upload any such health information. Neither PPHS nor HSS has any responsibility regarding the same, nor are we responsible for the quality of any x-rays, personal medical images, or other documents stored in MHAA.
You are responsible for choosing whether to open a MyHealth Alberta Account and to populate those records with health information. You do so entirely at your own risk. We are not responsible or in any way liable for your choices in respect of the same.
Choosing to Share Your Personal Health Information with Other MHR Users
Subject to the “Conditions Concerning Minors” below, you may choose to use MHAA’s functionality to grant access to other users to view your health information. By doing so, you are expressly providing consent and authorization to PPHS and HSS to allow those other registered users to access information within your MHAA in accordance with the permissions you have established via MHAA. You may share your information with other users for any purpose you wish.
This consent is effective as of the date that you issue an invitation to another user to view and/or modify information in your MHAA. Subject to the “Conditions Concerning Minors” below, you may revoke the other user’s access to information in your MHAA at any time.
Any sharing of information via MHAA is done entirely at your own risk. You acknowledge that you are aware of the risks and benefits of sharing or deciding to not share information in your MHAA with any other person.
Conditions Concerning Minors: For minors under the age of 16, as of February 2nd, 2026, a parent or guardian may request access to the minor’s health information without the minor’s consent using the online tools available through the MHAA. If the minor is over 16 but under 18 years of age, a parent or guardian may only request access through a health care professional. Once granted, the parent or guardian’s access to the minor’s health information cannot be revoked by the minor until they are at least 16 years of age, even if the minor creates their own MHAA. After the minor turns 18, parent/guardian access will automatically be removed. Minors should note that the choices you make about sharing information within MyHealth Alberta do not impact the ability of your parent or guardian to access your health information by requesting access online or from a medical services provider in certain circumstances. If you choose to share information in MHAA with a medical services provider, in some circumstances your parent or guardian may be able to access that information from that medical services provider or their own MHAA.
For all users, neither PPHS nor HSS is responsible or liable for:
- your choices in respect of this information sharing; or
- any collection, use or disclosure of your information by the person with whom you have shared your information (unless that person is acting as an affiliate of PPHS or HSS).
When you log into MHAA tools, a list of accesses made to your record by users to whom you have granted access to information in your account is electronically accessible for review. We recommend that you monitor this list on a regular basis. You agree to notify MHAA Support immediately of any suspicious access. Neither PPHS nor HSS is liable for any loss or damage arising from your failure to monitor your record or MyHealth Alberta account.
You hereby grant, and represent and warrant that you have the authority to grant us the right to access, transmit, receive, monitor, retrieve, store, maintain, and use your personal information, health information, and any other information you provide in connection with the establishment and maintenance of your MyHealth Alberta Account.
Indirect Collection of Health Information to Populate Personal Health Record
MHAA functionality enables you to have us populate your MHAA with certain aspects of your health information emanating from PPHS, HSS, or other third-party sources. The nature and extent of this information is solely within PPHS’s and HSS’s discretion to offer and/or provide to you and will be described in detail in MHAA. By agreeing to these terms of use, and to the extent any of this health information is not within the custody or under the control of either PPHS or HSS, you expressly authorize us to collect your individually identifying health information from the relevant third parties and to disclose it to you by entering it into your MHAA. We reserve the right to correct or amend any information we populate into your MHAA, including information obtained from a third-party source, to ensure the most recent or accurate information is available to you. You agree to flag any inaccuracies in the information populated into your MHAA through User Support.
Indirect Collection of Limited Aggregate Personal Information for Analytics Purposes
Further, MyHealth Alberta uses analytics to understand usage patterns, and enhance product capability and performance. PPHS and/or HSS may require limited aggregate personal information from third parties to meet the purposes. By agreeing to these terms of use, and to the extent any of this personal information is not within the custody or under the control of either PPHS or HSS, you expressly authorize us to collect your personal information from the third parties to understand usage patterns and enhance product capability and performance.
Confidentiality
We respect your privacy and seek to maintain the confidentiality of your health information and personal information in accordance with Alberta’s privacy legislations and our Privacy Policy, which are incorporated herein by reference. We will use your health information and personal information as necessary to provide services and operate MyHealth Alberta and MHAA, including but not limited to, converting documents received from you or on your behalf to an appropriate electronic format and maintaining your account. We will use and disclose it for no other purpose, other than in the following circumstances:
- we are required to do so by law, including by a subpoena, warrant or court order; or
- for the purpose of a court proceeding or a proceeding before a quasi-judicial body to which PPHS or HSS is a party.
By agreeing to these terms of use you indicate that you have read, understood, and accept our Privacy Policy. Except as otherwise stated, neither PPHS nor HSS owns or makes any claim to own any of the information in MHAA that you choose to upload, store, or create on MHAA.

The
Health Information Act and other legislation governing PPHS and/or HSS may permit discretionary disclosure of your health information without your consent; however, notwithstanding this, we expressly agree not to disclose any personal information or health information that you have entered or caused to be entered into your MHAA to any third party without your prior written consent or authorization, other than in the circumstances described above.
Accuracy & Integrity of Information
Although PPHS and HHS attempt to maintain the integrity and accuracy of MyHealth Alberta and MHAA, we do not make any guarantees whatsoever as to their integrity or accuracy. It is possible MyHealth Alberta and MHAA could include typographical errors, inaccuracies, or other errors, and that there may be unauthorized additions, deletions and alterations in the information that is made available to MyHealth Alberta and MHAA by third parties. In the event you discover an inaccuracy in your MyHealth Alberta account or MHAA, please inform MHAA Support so that it can be corrected. Any information contained in MyHealth Alberta and MHAA, other than the information you enter in your own MHAA, may be changed, or updated without notice.

MyHealth Alberta and MHAA may contain links or references to websites operated by other parties which might have information on health topics of interest to you. These links and references are provided for your general information and education only and should NOT be relied upon for personal diagnosis or treatment. We make no representations whatsoever about any other website that you may access through MyHealth Alberta or MHAA.
When you access a non-PPHS or non-HSS website, please understand that it is independent from PPHS and/or HSS, and that we have no control over the content on that website. In addition, a hyperlink to a non-PPHS or non-HSS website does not mean that we endorse or accept any responsibility for the content, or the use, of the linked site. It is up to you to take precautions to ensure that whatever you select for your use or download is free of such items as viruses, worms, Trojan horses, and other items of a destructive nature. If you decide to access any of the third-party websites linked to MyHealth Alberta or MHAA, you do this entirely at your own risk. We have no control over the security or privacy practices of these external websites. Use of other websites is strictly at your own risk. You are responsible for viewing and abiding by the terms and conditions of use and the privacy statements of the other websites.
MyHealth Alberta and MHAA can connect to certain third-party devices so that data from your devices may be uploaded into MHAA. Third party devices are owned and managed by third parties. Each device provider maintains its own privacy policy, End User Licence Agreement and/or other terms and conditions. Such terms and conditions govern your use of the third-party devices, connections, and associated software. Additional terms and conditions, including access to the third-party products, may be modified by the third parties in their discretion and as such we cannot guarantee your continued access to the devices. We make no representations that the third-party companies are compliant with data privacy legislation or other specific laws and regulations.

Termination, Suspension & Deactivation
PPHS and HSS reserve the right to change, suspend, or terminate your access to all or part of MyHealth Alberta or MHAA, or to deactivate any aspect of the MyHealth Alberta Account with or without cause, and with or without notice to you. This will also result in your no longer having access to any third-party services that you access using your MyHealth Alberta Account.
If we suspend, terminate, or deactivate your access to your MyHealth Alberta Account, your right to access any personal information or health information via MHAA shall also be suspended or terminated at that time. Other than as required by law or with your consent, we will not disclose or provide access to information stored in your MHAA after the termination, suspension, or deactivation of your MyHealth Alberta Account.
You may discontinue using MyHealth Alberta at any time. If you have created a MyHealth Alberta Account, you may formally deactivate your account at:

You may also delete the information in your MHAA at any time. Deleting MHAA information and/or deactivating your MyHealth Alberta Account will result in the information and the associated services no longer being accessible.
You understand and accept that your access to MyHealth Alberta and MHAA will be terminated by your request, upon your death, or if you do not access your account for a period of 24 months or more. We are not responsible for the loss or destruction of any of your information as a result of such termination.
Other than as required by law or with your consent, we will not disclose or provide access to information stored in your MHAA after you deactivate your MyHealth Alberta Account.
Limitation of Liability
We are neither responsible nor liable for any injuries (including wrongful death), direct, indirect, indirect, incidental, consequential, special, exemplary, punitive, or other damages whatsoever (including, without limitation, those resulting from lost profits, lost data, or business interruption) arising out of or relating in any way to the MyHealth Alberta Account, related services, content or information contained within the MHAA, health advice, diagnosis, treatment, the performance or possession of the MyHealth Alberta Account, any hyperlinked website, any defect in the MyHealth Alberta Account or its contents, and/or any breach of these terms of use. This limitation applies (a) whether the alleged claims are based on warranty, contract, tort, or any other legal theory and (b) regardless of whether or not we have been advised of the possibility of such damages.
Your sole remedy for dissatisfaction with the MyHealth Alberta Account, related services, and/or hyperlinked websites is to stop using MyHealth Alberta, MHAA and/or those services.
Disclaimer of Warranties
We make, and you shall receive, no warranties, expressed or implied, including any implied warranty of merchantability, fitness for a particular purpose, non-infringement or title. We do not warrant or represent that messages sent through MyHealth Alberta and MHAA will be read or responded to by your health services provider, or in a timely manner. We make no representation as to the reliability, accuracy, timeliness, completeness, or suitability of MyHealth Alberta, MHAA or any services made available through them.
Choice of Law & Venue
Although PPHS and HSS are in Alberta, MyHealth Alberta and MHAA can be accessed all across Canada, as well as from other countries around the world. Each of these places has laws that may differ from those of Alberta. By accessing MyHealth Alberta and/or MHAA, you agree the laws of the province of Alberta will apply to all matters related to these terms of use and the use of MyHealth Alberta and MHAA, without regard to conflicts of laws or principles thereof. Each party also agrees and hereby attorns to the exclusive personal jurisdiction and venue of the courts of the province of Alberta, and waives any jurisdictional, venue or inconvenient forum objections to such courts.
We make no representation that materials on MHAA or otherwise accessible with MyHealth Alberta are appropriate or available for use in other locations outside Alberta. Those who choose to access MyHealth Alberta and/or MHAA from other locations do so on their own initiative and are responsible for compliance with local laws.
Equitable Relief
You agree that monetary damages may not provide a sufficient remedy to us for violations of these terms of use and you consent to injunctive or other equitable relief for such violations.
International Use
By choosing to access the MyHealth Alberta and/or MHAA from any location other than Canada, you accept full responsibility for compliance with all local laws that are applicable. Neither PPHS nor HSS makes any representation that materials on MHAA or otherwise accessible with a MyHealth Alberta Account are appropriate or available for use in locations outside Canada and accessing them from territories where their contents are illegal is prohibited. You may not use, export or re-export any materials from MHAA or via MyHealth Alberta in violation of any applicable laws or regulations, including, but not limited to, any Canadian export laws and regulations.
Severability
In the event that any of these terms of use are held by a court or other tribunal of competent jurisdiction to be unenforceable, such provisions shall be limited or eliminated to the minimum extent necessary so that these terms of use shall otherwise remain in full force and effect.
Entire Agreement
These terms of use constitute the entire agreement between us and you pertaining to the subject matter hereof. Certain provisions of these terms of use may be superseded by expressly designated legal notices or terms located on particular pages within MyHealth Alberta and MHAA.
Indemnity
If you are 18 years old or older, you agree to indemnity and hold us harmless from any loss, liability, injury (including injuries resulting in death), claim, or demand, including reasonable legal fees and costs, of any kind due to, arising out of, or relating to your use of MyHealth Alberta, MHAA or content, including your use of MyHealth Alberta and/or MHAA to provide a link to another site or to upload content or other information to MHAA or otherwise, or your breach of the terms of use.
