Cancer Patient Navigation and Tumor Triage Coordination

This service supports clients and their families by:

  • providing accurate information for patients so they can make informed decisions about their next steps
  • helping patients prepare for tests and cancer treatments by explaining test results and treatment choices or by referring patients to other resources or services
  • improving coordination of care by working with other healthcare providers
  • helping patients prepare for medical visits by explaining ahead of time what to expect
  • helping patients access financial resources and community supports

Tumour Triage Coordinators focus their care to patients at the beginning stages of the cancer journey, but they can connect patients and families to other healthcare providers as needed.

Complex Cancer Management Service

This service operates on a consultative basis in conjunction with patients’ existing oncology and community care teams. Patients are seen by a physician or nurse practitioner. The team will follow the patients progress until symptoms are stable, and management is then continued by patients’ existing oncology and family medicine providers.

Department of Psychosocial and Spiritual Resources - Support Groups

All groups are professionally facilitated.  Groups bring together individuals who share a common experience.  In the group members may no longer feel alone or different.  They may realize that their reactions and emotions are a normal response to the challenges of a cancer diagnosis. 

A wide variety of support groups are offered with new sessions beginning in the fall, winter, and spring.

All requests are charged a fee in accordance with the Health Information Act. A basic fee of $25.00 is applied to all requests which includes up to 20 pages depending on record format i.e. paper, electronic or microfilm record. Patient Email Well Wishes

As part of our support for patient care and comfort, friends and family of inpatients at hospital and health care facilities can send well wishes via email. Sites participating in the program will receive the message, will ensure that the message is only "well-wish" (any other type of communication will be deleted) and will arrange for delivery within 3 business days. A couple of important reminders - the electronic delivery system is not a secure connection, so please don't add any confidential information to your message. Plus the "well-wish" system does not allow for replies or confirm receipt / delivery confirmation.

Psychosocial Oncology

The Department of Psychosocial Resources, through a team of trained professionals in psychiatry, psychology and social work, help patients and their families cope with the emotional, psychological and social stresses that often surface as a result of cancer and its treatment.  The team is available to patients from the moment of diagnosis onward.

Resource counselling for cancer survivors

  • three specially trained social workers called Resource counsellors help patients with their resource needs and deliver a weekly class for patients which includes infomation on how to get help for your financial and practical needs.

Taught by a nurse who specializes in working with radiation therapy patients, the class focuses on the unique experience of breast cancer treatment, possible side effects, and the effective management of these side effects, if they should occur.

Spiritual Care Services

This Service Provides spiritual and emotional support for patients, families and staff as an integral part of the multidisciplinary care team that works within the health care system to assess and address spiritual care needs, which may include:

  • spiritual assessment and intervention
  • facilitating rituals and sacraments (e.g. sweet-grass, sacrament of the sick)
  • decision-making support and supportive care for patient and family at end-of-life
  • collaboration with religious leaders at patient's and/or family's request
  • serving as a resource to care team on topics such as communication, religious and cultural awareness, grief and bereavement, compassion fatigue, and spiritual needs assessment
  • daily 24-hour (on-call) coverage at most acute care centres
