How do I get a copy of my immunization records?
If you received immunizations in Alberta, you can get many immunization records online or at a public health or community health centre by visiting Get immunization records.

How do I get my medical records?
To get a copy of your medical records from your doctor, contact your doctor’s office. For your health records for services or procedures you’ve had at an Alberta Health Services facility or clinic, contact AHS Access & Disclosure.

If you’re signed up for MyHealth Alberta Account, you can also get some of your health records through My Personal Records or MyChart.

What if I need technical support or have questions about my MyHealth Alberta account?
