The Allergies tool allows you to keep track of your allergies. By clicking the “red button” in the bottom right, you can add an allergy.

You can add the type of allergic reaction (such as hives, rash, itching, tongue and throat swelling, or vomiting), date and time of the reaction, what to do, and other important notes.

Enter all required information, select from the drop-down list, and click “Save” when finished. 

You can view, edit, or delete an allergy listed by clicking on the allergy at any time.

The Appointments tool allows you to keep track of your past and upcoming appointments.  By clicking the “red button”  , you can note who the appointment is with, the reason for the appointment, date and time, and any other important details. Click “Save” when you are done.

You can view, edit, or delete an appointment by clicking on the appointment at any time.

 

The Family History tool lets you track information about your family’s health history. This is helpful in monitoring health conditions early or preventing you from developing a health condition that you may be at risk of getting.

Click the “red button” to add a family member, their relationship to you, their health condition, and other important notes.

Click on a family member in the list to edit or delete their health information. You can add more conditions for a family member when you choose them from the list, select “Edit,” and then “Add conditions.” Once completed, click “Save” and close.

The Immunizations tool helps you keep track of all your immunizations given through public health services in Alberta. This information is updated frequently. Contact MHR Support at 1-844-401-4016 if your information is wrong or not up-to-date.

The “Set up Notifications” blue button allows you to receive updates about your Immunization record or send notifications to someone you trust. If you want someone you trust to get notifications about your immunizations, share your record with them first so they can see your updates.

You can see more information by clicking the small “information icon” button . Click on the card to open into a new screen so the information is easier to read.

The blue "Print Immunizations" button allows you to create and print a PDF report of your information.

Click on “Print Immunizations,” select your preferred date range (all days or anywhere between 7 days to 12 months), and save your selection.

You can also select what type of information you want to see printed on your report (allergies, appointment information, blood glucose, etc.)

When your selections have been made, click the blue “Export” button to produce the report.  The PDF report will be created and you can save anywhere on your device.

Once you have made your selection, click "Save".

My Personal Records users have access to test results ordered from all sites in Alberta, including labs obtained during inpatient and emergency visits.

Your immunization records are usually available within 24 hours. Make sure your Alberta.ca Account verification process is complete to continue receiving up-to-date records. Your verification status is available on your MyHealth Records dashboard in the Identity Verification tab. If you are not verified, follow the Alberta.ca Account verification instructions.

The Medical Conditions tool allows you to keep track of your health conditions, such as asthma, diabetes, and high blood pressure. 

Click the “red button”  to add a medical condition, the date of diagnosis, treatments, and other important notes. As you begin typing the medical condition, common associated words will appear and you can select or self-enter your own.

You can edit or delete a medical condition by clicking on the drop-down list. If you no longer have a certain medical condition, click the “No longer have this medical condition” box. Enter the diagnosed date, time and any additional notes you want to include.

You will see two sections in the Medications tool.

When you start using MyHealth Records, the Alberta Netcare Prescription Medications section will show your prescriptions for the last 18 months. Newly dispensed prescriptions from most pharmacies show up every time you login.

Updates to this section will show your prescriptions up to 11:59 p.m. as of the date and time indicated. You cannot change or edit this information. Contact MHR Support at 1-800-315-6028 if your information does not look right.

Medication records from Alberta Netcare are updated every day.

Dispensed Prescription Medications

This section shows your medications which includes:

• Medication information
• Your refills

The name of any drug that has been discontinued will be struck through.

You can see more information by clicking the small "information icon" button . You can also click on a specific medication that appears in your record. This will open into a new screen so the information is easier to read.

The blue "Set up Notifications" button allows you to receive updates about your Medications or send notifications to someone you trust. If you want someone you trust to get notifications about your medications, share your record with them first so they can see your updates.

Other Medications

You can add other prescriptions, over-the-counter medications, and vitamin or herbal supplements.

You can add medications by clicking on the "red button" 

You can enter the following information:

• Medication name
• Date the prescription was filled
• The day you started your over-the-counter medication, such as vitamin or herbal supplement
• Strength (such as 500 mg)
• Dosage (such as 1 tablet or 2 drops)
• Frequency (such as once a day)
• Route (for example by mouth or inhaler)
• Date you discontinued taking the medication

You can click on any medication in the list to edit or delete it.

The blue "Set up Notifications" button allows you to receive updates about your medications or send notifications to someone you trust. If you want someone you trust to get notifications about your medications, share your record with them first so they can see your updates.

Adding your Mobile Health to your Alberta Wallet

• Sign in to your MyHealth Records and select the My Personal Records tile.

• Select the Mobile Health Card tile from Health Summary.

• Select “Add to Alberta Wallet”.

• Your Mobile Health Card is now added. Select a card to view the QR code.

To view and use your card:

1. Log in to your Alberta.ca Account.
2. Select Alberta Wallet.
3. Select a Mobile Health Card to view the QR code.

Sharing your Mobile Health Card with someone you trust

If you are 14 years or older, you can share your Mobile Health Card with someone you trust through your MyHealth Records account.

Common examples of whom people often share their Mobile Health Card with:

• Parents or grandparents
• A spouse or partner
• Children who are 14 years or older
• Anyone else you trust to share your health information with

How to share your Mobile Health Card with someone you trust.

There are 2 parts to this process:

Part 1:

To share your Mobile Health Card and health information with a person you trust, follow the steps below. This part of the process will be completed by you.

Part 2:

The second part is completed by the trusted person who you have shared your Mobile Health Card with. The person must accept your invitation to share your Mobile Health Card, then add it to their Alberta Wallet. Steps are listed in the next section “Adding someone’s Mobile Health Card to your Alberta Wallet.”

Follow the steps below to share your Mobile Health Card and health information with someone from your MyHealth Records account.

• Sign into your MyHealth Records account.
• Click on My Personal Records.
• Select the Share My Record tool on your Home page, on the Tools and Settings page, or from your Profile icon in the top right corner of the page.

• Click the ‘Share health information with someone you trust’ button.

• Enter the email address of the person (i.e. recipient) you are sharing your record with. You will need to do this twice to confirm the address is correct.
• Enter a passcode. The recipient needs to enter this passcode when they accept your invitation. Choose a passcode between 6 to 10 characters. You can use numbers and uppercase or lowercase letters. Do not use special characters such as #, %, or punctuation. Share your passcode with the recipient in a secure way such as a phone call.
• Choose a sharing level. The sharing level describes how much access you are giving to the person you are sharing your record with.
• Choose the types of information in My Personal Records that you would like to share with the person.

• Choose an access expiration date. On this date, the recipient’s access to your record ends, and they can no longer see your record. You can also choose no expiration date.
• Click ‘Send Invitation’.

The recipient will then receive an email with a link to accept the sharing of the health record. Once they click the link, the recipient needs to enter the passcode to accept your invitation.

After accepting the invitation, the recipient will be able to add your Mobile Health Card to their Alberta Wallet by following the steps mentioned in the section below “Adding someone’s Mobile Health Card to your Alberta Wallet.”

Adding someone’s Mobile Health Card to your Alberta Wallet

To add someone’s Mobile Health Card to your Alberta Wallet, ensure they have first shared their health record with you. Follow the steps in the above section “How to share your Mobile Health Card with someone you trust.”

Once someone has shared their Mobile Health Card with you, you will receive an email invitation. Adding the card to your Alberta Wallet requires a passcode that the person will provide to you.

The email invitation is valid only for 24 hours. If the invitation expires before you can accept it, another invitation can be shared.

Follow the steps below:

• Sign in to the Alberta Wallet using your Alberta.ca account login credentials.
• Click “Add to Alberta Wallet” button.

• Select “Get a Mobile Health Card.”

• Select the health record for the person whose card you want to add to your Alberta Wallet.
• Click “Add to Alberta Wallet” to add their Mobile Health Card.

You will be then redirected back to the Alberta Wallet and can access their Mobile Health Card.

Downloading a shared Mobile Health Card

You may download the Mobile Health Card for people who have shared their mobile health card on their health record with you, such as your parent, spouse or children.

What is a shared record

Your personal health record can be shared with someone you trust. You control the kind of information you share and what the person you share with, can do with your information. Learn more.

• Select the name of the person whose card you would like to add from the box in the top right corner.

• Follow the same steps above to download the Mobile Health Card from a web browser.

Adding a child’s Mobile Health Card for children under 14

You can request Mobile Health Cards for your children by verifying your relationship.

• Follow the prompt to log in to MyHealth Records.
• On the dashboard, select the "Children's Mobile Health Card" tile.

• Select "Add a child’s Mobile Health Card".

• Enter your child’s information and affirm the declaration, then click “Submit.”

• If your request is successful the card will be issued right away and be available in your Alberta Wallet.

What to do if we are unable to validate your child’s information

If you see the message "Unfortunately, we were unable to validate the information you submitted" while trying to add your child’s Mobile Health Card, it means we could not confirm their details automatically.

This may happen if:

• Your family information has not been updated in our system
• Your child is not linked to your Alberta Health Care Card Insurance Plan account
• Custody or guardianship needs additional verification

You can add your child’s Mobile Health Card by uploading documents that validate your relationship.

Step 1: Gather your supporting documents

To validate your relationship to the child, you must upload at least one of the following:

• Child’s long form birth certificate showing your name as a parent
• Adoption records
• Court order (e.g., custody or guardianship)
• Separation or divorce agreement showing parental responsibility
• Other legal documents proving your relationship

Note: Take a clear photo or scan your documents and ensure all names, dates, and signatures are easy to read.

File upload requirements:

Accepted file types: .jpg, .jpeg, .png, .pdf.
File size: Up to 10 MB per file.
Upload limit: Maximum 5 files per submission.

Step 2: Subject your request

On the file upload page:

• Select your relationship with the child from the dropdown.
• Enter your phone number.

Upload your documents:

• Drag and drop the files into the upload area or click to select them from your device.

Click submit files to send your request.

What happens next?

After you click Submit files, you will see a confirmation message.

Confirmation code

You will receive a confirmation code, keep this code for your records in case you need to follow up about your request.

Confirmation email

You will receive an email confirming receipt of your request. This email will include a confirmation code for your records and information on what to expect next.

We will contact you

Once our team has reviewed your documents, we will send you a follow-up email to let you know the outcome and whether your child’s Mobile Health Card has been added.

To remove a Mobile Health Card from your Alberta Wallet:

1. Navigate to your Alberta Wallet.
2. Select the ellipsis in the top right corner.
   
3. Select 'Remove.'

The Referrals tool in My Personal Records (MPR) enables you to view information for some referrals submitted on or after April 26, 2025. If you do not see your referral in My Personal Records or MyChart, it may have been sent directly from your health care provider to the specialist instead of through the provincial referral source systems. All reports are released to users immediately once data is available.

A referral is a request from your health care provider to another provider, often a specialist, asking for advice or help with your care.

Referral requests go through many steps and usually end with a scheduled appointment with the specialist. Information about the status changes for your referral may be available in MPR.

Referrals List

Available referral information appears in a table.

Last Updated	The date when a change was last made to the referral. This date is updated when there is a change to the status, status reason, etc.
Date Submitted	The date the referral was sent from your health care provider to the specialist.
Referral Type	All referrals display as Referral.
Referral Reason	The specialty/ sub-specialty of the health care provider that is receiving the referral. If your referral was submitted on or after April 26, 2025, you may see it in the list.
Status	The status of the referral. The status is updated as the referral progresses through the process from when the referral is created by a health care provider to when you attend the first appointment with a specialist.
Status History	Enables you to view all changes in referral status with the most recent status listed first.

Referral Details

Click on a row in the table to open the ‘Referrals’ window and view more information.

The Referrals window provides the following information:

Referral ID	This is a 12-character identifier made of up letters or numbers that is used for identifying the referral request. You may be asked for this number if you call MyHealth Records Support as it can help the support agents in responding to your questions.
Date Submitted	The date that your health care provider sent the referral to the specialist.
Referral Type	All referrals display as Referral.
Referring Provider Name	The name of the health care provider who submitted the referral on your behalf.
Status	The status of the referral. The status is updated as the referral progresses through the process from when the referral is created by the provider to when you attend the first appointment with a specialist.
Status History (button)	Enables you to view all changes in the referral status with the most recent update listed first.
Referral Status Reason	Provides details about the referral status and more information on why a status is assigned to a referral. For example, if the status is ‘Reassigned’ the status reason may say “Being assigned to provider” or if the status is ‘Cancelled’ the status reason may be ‘Referral no longer needed’ or ‘Patient is not yet ready for specialist care.’
Received By	This is the service area that receives and assesses the referral documents to ensure they are complete and monitors the progress of the referral.
Referral Status Reason	The specialty/ sub-specialty of the health care provider that is receiving the referral.
Last Updated	The date when a change was last made to the referral. This date is updated when there is a change to the status, status reasons, etc.

View your Status History by clicking the ‘Status History’ button. All changes in your referral status are shown with the most recent update listed first.
